Feeding your baby:
Please feed your baby 2-2 1/2 hours prior to your appointment. At the time of your consultation we would like baby to be approaching his or her usual feeding time. The consultation will go more smoothly if your baby is not overly hungry. This extra time will also allow us time to discuss your concerns and get a detailed history prior to the feeding.
We ask that you and your baby wear comfortable clothing to your appointment. Consider clothing options that are easy to wear while feeding your baby.
At the time of your appointment we will provide verbal and written education, breastfeeding tools (as needed), resources as well as hands on breastfeeding support.
At the time of your appointment you will be given a personalized care plan. Your care plan will include individualized instructions to help you reach your breastfeeding goals.
Physician information with your consent:
With consent your feeding assessment will be faxed to your baby’s primary and/or referring care provider. We will also discuss findings with your baby’s care provider on an as needed basis.
All pre-consultation and new patient forms are available online under the ‘Forms and Documents’ section. Filling out paperwork online allows us to be well prepared for your consultation. If you are unable to fill this paperwork out ahead of time we ask that you arrive 10-15 minutes prior to your scheduled appointment time to complete the required paperwork. Consents to treat you and your baby will be signed at the time of your appointment.
We require a $15 deposit to schedule consultations online. This deposit will be deducted from the cost of your consultation when payment is collected.
